How to RegisterThis site is only available to existing patients who are age 0 and over.If you are a current patient of ours you can Register Online This will allow you to book appointments at our clinics.If you are not currently a patient of ours, please print out the Portal Consent Form and bring it to our clinic with your Health Card. If your health card does not have a photo, you must also present another government issued card that includes a photo ID. 1.
Register Online to book an appointment. You must be a current patient of ours. New patients, please visit any of our clinics to register. A limited range of features is available until you are fully registered.
2.
Please fill out the consent form and bring it into our clinic with your health card. If your health card does not have a photo, you must also present one piece of government-issued photo ID.
3.
Once you have completed the consent form with us, you will have access to the full range of features on the Patient Portal.
